Madison Metropolitan School District Superintendent Art Rainwater has released findings of an investigation as to how the wrong ballots got printed for the recent referenda vote.
The report says the wrong ballot information was printed by the city clerk, because of a series of miscommunications.
That information was supplied by Roger Price, secretary of the Board of Education.
Rainwater says the initial error was made by the school district's bond counsel.
The superintendent's office says no disciplinary action will be taken in the case.
