MADISON -- Wisconsin Department of Regulation and Licensing Secretary Celia Jackson today announced that the following final decisions and orders were issued in May 2009:
Thomas R. Anderson – Dentist, Waterloo. The Dentistry Examining Board reprimanded Thomas R. Anderson, and ordered him to pay costs of $700. Anderson must also complete remedial education in the area of the scope of dentistry practice and delegation. Anderson delegated the performance of dental examinations to dental hygienists and billed for dental exams that were not performed.

Joseph L. Arena – Dentist, Waterloo. The Dentistry Examining Board reprimanded Joseph L. Arena, and ordered him to pay costs of $700. Arena must also complete remedial education in the area of the scope of dentistry practice and delegation. Anderson delegated the performance of dental examinations to dental hygienists and billed for dental exams that were not performed.

Julie A. Birtzer – Registered Nurse, Chippewa Falls. The Board of Nursing revoked the license to practice as a registered nurse of Julie A. Birtzer, and ordered her to pay costs of $994.23. Birtzer may apply for a license as a registered nurse after one year upon a showing, among other things, that a psychological/psychiatric evaluation indicates that she can safely engage in the practice of professional nursing. Birtzer was convicted of 2nd degree intentional homicide.

Robert A. Brevard – Dentist, Montello. The Dentistry Examining Board accepted the surrender of Robert A. Brevard’s license to practice dentistry, and ordered him to pay costs of $100 prior to re-registration. While employed by the Department of Corrections Brevard failed to respond to dental service requests, shredded dental service requests and failed to document treatment provided in patients’ dental records.

Susan A. Buchholtz – Real Estate Appraiser, Lake Mills. The Real Estate Appraisers Board reprimanded Susan A. Buchholtz, and ordered her to pay costs of $850. Buchholtz must also complete remedial education in valuation of real estate and liability or quality assurance for appraisers, and she may not supervise other appraisers or appraise more complex properties than single family dwellings until approved by the Board. Buchholtz failed to comply with USPAP in appraisals and appraisal reports.

Beatta K. Christoffersen – Real Estate Appraiser, Appleton. The Real Estate Appraisers Board reprimanded Beatta K. Christoffersen, suspended her for a period of 40 days, and ordered her to pay costs of $1,000. Christoffersen must also complete remedial education in real estate valuation and ethics. Christoffersen performed appraisals and rendered an appraisal report that did not conform to USPAP.

Francine L. Cosner – Physician, Port Washington. The Medical Examining Board reprimanded Francine L. Cosner, and ordered her to pay costs of $955. During surgery Dr. Cosner perforated a patient’s bowel, mistakenly thought she’d perforated the uterus, and then, after the patient developed symptoms of the perforated bowel, the Board determined that Dr. Cosner was not as responsive as she should have been.

Janet M. Cruz – Registered Nurse, Sparta. The Board of Nursing suspended indefinitely the license and privilege under the Nurse Licensure Compact to practice as a registered nurse of Janet M. Cruz, and ordered her to pay costs of $1,900. Cruz may petition for a stay upon a showing that she has complied with drug and alcohol treatment and monitoring programs, and has abstained from personal use of alcohol and controlled substances. Cruz shall not work as a health care provider in a setting in which she has access to controlled substances, nor work in a home health care, hospice, pool nursing, or agency setting, and shall practice only under the direct supervision of an approved health care professional. Cruz was found to be medically impaired by multiple medical conditions.

Marc D. Ditty – Chiropractor, Green Bay. The Chiropractic Examining Board limited the license to practice chiropractic, and ordered him to pay costs of $305. Ditty must complete remedial education in patient record keeping and case management. Ditty failed to maintain a copy of a patient’s MRI and its evaluation, failed to document his clinical observations of the patient and the opinion offered to the patient concerning the MRI.

Victoria Fetter – Physician, Milwaukee. The Medical Examining Board reprimanded Victoria Fetter, and ordered her to pay costs of $2,188. Fetter must complete remedial education in psychiatrist-patient confidentiality, confidentiality of mental health treatment records and HIPAA. Fetter divulged privileged communication and information entrusted by a patient.

Kristi Ann Fitzgerald – Registered Nurse, Hewitt. The Board of Nursing reprimanded Kristi Ann Fitzgerald, and ordered her to pay costs of $640. Fitzgerald must also complete remedial education in medication management administration and charting. While working as a registered nurse Fitzgerald failed to properly account for and document administration of controlled substances.

John T. Friedrichs – Chiropractor, Pewaukee. The Chiropractic Examining Board reprimanded John T. Friedrichs, and ordered him to pay costs of $755. Friedrichs must also provide a written disclosure to patients that he intends to treat with the DRX9000 device for vertebral decompression. Friedrichs engaged in advertising in a manner which is false, deceptive and misleading.

Megan L. Frohmader – Registered Nurse, Waukesha. The Board of Nursing suspended indefinitely the license and privilege under the Nurse Licensure Compact to practice as a registered nurse of Megan L. Frohmader, and ordered her to pay costs of $300. After six months Frohmader may petition for a stay upon a showing that she has complied with drug and alcohol treatment and monitoring programs, and has abstained from personal use of alcohol and controlled substances. Frohmader may not work as a health care provider in a setting in which she has access to controlled substances, nor may she work in a home health care, hospice, pool nursing and agency setting. She must work under the direct supervision of an approved health care professional. Because of her behavior and appearance while working as a nurse, Frohmader was requested to take a drug test by her employer. She tested positive for high levels of morphine, oxycodone, oxymorphone and unprescribed Dilaudid.

Carol A. Garber-Hintze – Registered Nurse, New Berlin. The Board of Nursing revoked the license to practice as registered nurse of Carol A. Garber-Hintze, and ordered her to pay the full costs of the proceeding. Garber-Hintze diverted controlled substances for her personal use while working as a professional nurse.

Gary R. Gienapp – Veterinarian, Chippewa Falls. The Veterinary Examining Board reprimanded Gary R. Gienapp, and ordered him to pay costs of $650. Gienapp must also complete remedial education in the care of geriatric companion animals, and client patient informed consent, veterinary record keeping and patient care documentation. Gienapp failed to recognize that a cat’s symptoms of no vision, rough waking up, and thrashing as symptoms of an anesthetic reaction, failed to appropriately evaluate, assess, and treat that condition, and failed to record and document vital signs, to provide diagnosis, to provide written post-operative care instructions, and to obtain written consent or written refusal for testing.

Richard Givhan – Registered Nurse, Milwaukee. The Board of Nursing limited the license of Richard Givhan to practice as a registered nurse in that he may not provide clinical nursing services until such time as he can demonstrate that he is physically capable of practicing clinically with skill and safety to patients. Givhan is suspected of working as a registered nurse while intoxicated, and is currently unable to provide clinical nursing services by reason of a medical condition.

Wayne Robert Griffin – Veterinarian, Chippewa. The Veterinary Examining Board reprimanded Robert Griffin, and ordered him to pay costs of $650. Griffin must also complete remedial education in the area of pre-operative assessment for administration of anesthesia and post-operative care of companion animals, and veterinary client patient informed consent, record keeping and patient care documentation. Griffin, in the treatment of a cat, administered both a steroidal medicine and a non-steroidal medicine at same time, which was contraindicated, failed to recognize the symptoms of an anesthetic reaction and to appropriately evaluate, assess, and treat that condition, and failed to properly document vital signs, to provide diagnosis, written post-op care instruction, and to obtain written consent or written refusal for testing.

Don J. Grogan – Real Estate Appraiser, Marion. The Real Estate Appraisers Board revoked the license to practice as a real estate appraiser of Dan J. Grogan, and ordered him to pay the full costs of the proceeding. Grogan failed to comply with the USPAP in appraisals and appraisal reports.

Michael M. Hamilton – Chiropractor, Green Bay. The Chiropractic Examining Board reprimanded Michael M. Hamilton, and ordered him to pay costs of $570. Hamilton must also publish in his local news paper and distribute to patients a disclaimer that chiropractic care cannot cure cardiac health problems, and he must complete remedial education in patient record keeping and case management. Hamilton advertised in a manner which was false, deceptive and misleading, and failed to maintain a copy of a patient’s MRI, to document clinical observations and to document the opinion offered to the patient of the evaluation of the MRI.

Aimee Hamilton – Chiropractor, Green Bay. The Chiropractic Examining Board limited the license of Aimee Hamilton in that she must complete remedial education in patient record keeping and case management, and ordered her to pay costs of $305. Hamilton failed to maintain a copy of a patient’s MRI, to document clinical observations and to document the opinion offered to the patient of the evaluation of the MRI.

Jon C. Hanson – Agent for Burial Agreements, Franksville. The Funeral Directors Examining Board limited the license of Jon C. Hanson to practice as an agent for burial agreements in that he may not have unsupervised contact with minors at any time, and ordered him to pay costs of $1,000. Hanson was convicted of transporting child pornography the circumstances of which are substantially related to the practice of funeral directing and acting as an agent for burial agreements.

Michelle Lynn Hodge-Cowan – Registered Nurse, Onalaska. The Board of Nursing suspended for an indefinite period the license to practice as a registered nurse of Lynn Hodge-Cowan, and ordered her to pay costs of $600. Hodge-Cowan may petition for a stay upon a showing that she has complied with alcohol and drug treatment and monitoring programs, and abstained from personal use of alcohol and controlled substances. Hodge-Cowan shall not work as a health care provider in a setting in which she has access to controlled substances, nor in a home health care, hospice, pool nursing or agency setting, and may only practice under the direct supervision of an approved health care professional. Hodge-Cowan diverted controlled substances from patients to her personal use, and was convicted of possession of a controlled substance without a valid prescription.

Gerald James House – Practical Nurse, Madison. The Board of Nursing suspended for an indefinite period the license and privilege under the Nursing Licensure Compact to practice as a practical nurse of Gerald James House, and ordered him to pay costs of $100. House may petition for a stay upon a showing that he has complied with alcohol and drug treatment and monitoring programs, and abstained from personal use of alcohol and controlled substances. House shall not work in a home health care, hospice, pool nursing or agency setting, and may only practice under the direct supervision of an approved health care professional. House has a history of alcohol dependence and cognitive impairments.

Benjamin F. Ingaldson – Dentist, Greenfield. The Dentistry Examining Board reprimanded Benjamin F. Ingaldson, and ordered him to pay costs of $425. Ingaldson, mistaking the numbering system used by orthodontists for the numbering system used by general dentists, extracted the wrong teeth of a patient.

Patrick J. Crawford – Dentist, Kenosha. The Dentistry Examining Board suspended for one week the license to practice dentistry of Patrick J. Crawford, and ordered him to pay costs of $2,478. Crawford’s license to practice dentistry is also limited in that he must not perform any endodontic treatment on any patient until he has completed remedial education in acceptable billing practices, ethics of dental practice, and patient documentation and dental record keeping. Crawford attempted to obtain compensation by fraud or deceit in his dentistry practice, and failed to notify a patient that he perforated a tooth during a root canal treatment, and failed to repair the perforation or refer the patient to another dentist to have the perforation repaired.

Steven J. Jagemann – Dentist, Hales Corners. The Dentistry Examining Board summarily suspended the license to practice dentistry of Steven J. Jagemann. Jagemann has been charged with felonies upon the discovery of marijuana plants in a house owned and controlled by him, and allegedly operated his dental office in an unclean, unsanitary and unhealthy manner.

Pamela A. Johnson – Real Estate, Austin, Texas. The Department of Regulation and Licensing enjoined Pamela A. Johnson from practicing as a real estate broker without a license. Johnson was involved in the sale of real estate properties in Wisconsin without obtaining a real estate license.

Cynthia L. Kingsbury – Registered Nurse, Mauston. The Board of Nursing ordered Cynthia L. Kingsbury, a licensed registered nurse, to complete remedial education in practicing within the scope of practice permitted by law, including proper administration of controlled substances. Kingsbury must also pay costs of $320. Kingsbury administered a medication to a patient without a physician’s order.

Joel P. Mayberry – Veterinarian, Chippewa Falls. The Veterinary Examining Board reprimanded Joel P. Mayberry, and ordered him to pay costs of $650. Mayberry must also complete remedial education in the areas of pre-operative assessment for administration of anesthesia, post-operative care of companion animals, care of geriatric companion animals, veterinary client patient informed consent, and veterinary record keeping and patient care documentation. Mayberry failed to recognize a cat’s symptoms as that of an anesthetic reaction and to properly evaluate, assess and treat that condition, and failed to record and document vital signs, to provide a diagnosis, to provide post-op care instructions and to obtain written consent or written refusal for testing.

Kate S. Olson – Registered Nurse, Slinger. The Board of Nursing reprimanded Kate S. Olson, and ordered her to pay costs of $600. Olson’s license to practice nursing is limited in that she must submit to drug and alcohol testing upon request of the Board or the Department or other investigators. Olson took doses of a controlled substance from her place of employment without having a prescription or other authority.

Terrence J. Osmanski – Registered Nurse, Wausau. The Board of Nursing revoked the license to practice as a registered nurse of Terrence J. Osmanski, and ordered him to pay the full costs of the Department’s investigation and prosecution. Osmanski, while working as a nurse, diverted controlled substances. He failed to report all his convictions to his employer and to the Department, and he continued to work as a nurse after his license renewal was denied.

Mark S. Pichelmeyer – Funeral Director, Wausau. The Funeral Directors Examining board revoked the funeral director license of Mark S. Pichelmeyer, and ordered him to pay the full costs of the investigation and prosecution. Pichelmeyer has convictions for possession of controlled substances and resisting or obstructing an officer.

Tami L. Ponshock – Practical Nurse, Antigo. The Board of Nursing suspended for an indefinite period the license and privilege of Tami L. Ponshock to practice as a practical nurse, and ordered her to pay costs of $245. After 6 months Ponshock may petition the Board for a stay upon a showing that she has complied with drug and alcohol treatment and monitoring programs, and has abstained from personal use of alcohol and controlled substances. Ponshock shall not work as a health care provider in a setting in which she has access to controlled substances, shall practice only under the direct supervision of an approved health care professional, and she may not work in home health care, hospice, pool nursing, or agency setting. Ponshock diverted controlled substances from her place of employment, and obtained prescription medications without prescription from a person for whom the medications were prescribed.

Scott J. Schneider – Real Estate Appraiser, Green Bay. The Real Estate Appraisers Board reprimanded Scott J. Schneider, and ordered him to pay costs of $2,000. Schneider must also complete remedial education in real estate appraisal. Schneider’s license to practice real estate appraisal is limited in that he shall not provide training to, nor supervise, other appraisers or any individuals obtaining appraisal experience. Schneider performed appraisals and rendered appraisal reports that did not conform to the USPAP.

Sarah A. Spaeth – Registered Nurse, South Milwaukee. The Board of Nursing summarily suspended the license to practice nursing of Sarah A. Spaeth. While employed as a nurse repeated irregularities with controlled substances were attributed to Spaeth, and she tested positive for a number of controlled substances.

Ann E. Stanton – Registered Nurse, Manitowoc. The Board of Nursing suspended for an indefinite period the license and the privilege under the Nurse Licensure Compact as a registered nurse of Ann E. Stanton, and ordered her to pay costs of $300. Stanton’s suspension is stayed based upon a showing that she has complied with drug and alcohol treatment and monitoring programs, and has abstained from personal use of alcohol and controlled substances. Stanton shall not work as a health care provider in a setting in which she has access to controlled substances, shall practice only under the direct supervision of an approved health care professional, and she may not work in home health care, hospice, pool nursing, or an agency setting. While working as a nurse, Stanton diverted controlled substances to her personal use.

Christopher J. Suick – Real Estate Appraiser, Deerfield. The Real Estate Appraisers Board reprimanded Christoper J. Suick, and ordered him to pay costs of $700. Suick must also complete remedial education in real estate appraisal. Suick performed appraisals and rendered an appraisal report that did not conform to the USPAP.

Jacques R. Thebert – Veterinarian, Franklin. The Veterinary Examining Board reprimanded Jacques R. Thebert, and ordered him to pay costs of $1,100. Thebert must complete remedial education in veterinary medical documentation and record keeping. When a client requested a radiograph of her dog to aid a second opinion, Dr. Thebert gave her a radiograph, mislabeled; when Dr. Thebert was requested to produce his records for the Department’s investigation, he was initially unable to locate either those records or any indication that he had ever had such a client or patient.

Jonathan W. Thomas – Physician, Green Bay. The Medical Examining Board summarily suspended the license of Jonathan W. Thomas to practice medicine and surgery. There is probable cause to believe that Thomas was having sexual contact with patients.

Jung-Nan Tsai – Physician, Marinette. The Medical Examining Board accepted the offer by Jung-Nam Tsai to let his license lapse. Tsai failed to repeat a complete blood count, or diagnose and treat the infection, or both, of an infant with a significantly elevated white blood cell count of 26,700.

Gregory L. Vordermann – Real Estate Appraiser, Monroe. The Real Estate Appraisers Board reprimanded Gregory L. Vordermann, and ordered him to pay costs of $550. Vordermann must also complete remedial education in real estate appraisal. Vordermann performed appraisals which did not conform to the USPAP.

Joan A. Wegner – Registered Nurse, Franklin. The Board of Nurse suspended for an indefinite period the license and privilege under the Nursing Licensure Compact to practice as a registered nurse of Joan A. Wegner, and ordered her to pay costs of $450. After 3 months Wegner may petition the Board for a stay upon a showing that she has complied with drug and alcohol treatment and monitoring programs, and has abstained from personal use of alcohol and controlled substances. Wegner has a history of alcoholism.

Robert E. Will – Real Estate Appraiser, Las Vegas, Nevada. The Real Estate Appraisers Board suspended for an indefinite period the license to practice real estate appraisal of Robert E. Will, and ordered him to complete remedial education in real estate appraisal. Will must also pay the costs of the proceeding. Will rendered an appraisal report that did not conform to the USPAP.

James P. Williams – Registered Nurse, Mesa, Arizona. The Board of Nursing issued an interim suspension of the license to practice professional nursing of James P. Williams. The Department is investigating a report that Williams had sexual contact with a patient in his care.

I think it is terrible that Google, which is seen all over the world, would allow State of Wisconsin information for individuals licensed to practice to be shown by just typing their name. If residents of Wisconsin want information about an individual, they should only be able to get it from the State of Wisconsin website. It is not right to broadcast this around the world. It is a totally unfair and unbalanced judgement and borders on slander.
To Anonymous @12:18 - Only a small handful of medical professionals have a problem with substance abuse. The vast majority are competent, well trained and dedicated professionals who will treat you and everyone else with care and expertise.
WOW.... Alot of drunk and stoned people working in hospitals.
