From the Green County Sheriff's Office:
At 9:52 P.M. (October 24, 2012) Deputies, Wisconsin State Patrol, Green County EMS and Browntown First Response responded to a two vehicle crash in the W8200 block of STH 11, Town of Cadiz.
Investigation revealed that Cory J. Czechowski, age 27 of South Wayne, was westbound on STH 11 when he crossed the centerline and side swiped an eastbound vehicle that was operated by Tyler A. Plath, age 19 of Warren, IL.
Plath's vehicle then became disabled in the middle of the road.
Cory's vehicle continued west on STH 11, struck the guard rail on the south side of the road, then continued west into the south ditch and struck a utility pole.
A passenger in Cory’s vehicle, Steven T. Czechowski, age 22 of South Wayne, was wearing his seat belt and was transported by EMS for treatment of injuries sustained in the crash.
At the scene Steven claimed he was driving, but it was later determined that Cory was driving. Cory was not wearing his seat belt, sustained injuries in the crash but refused transport for treatment.
The air bags did not deploy in their vehicle.
Cory was arrested for OMVWI 1st offense. Cory was also cited for left of center, operating a motor vehicle with no insurance, failure to wear seat belt, and operating with a prohibited alcohol concentration. Cory's vehicle sustained very severe damage and was towed from the scene. Steven was cited for Obstructing an Officer and was released pending court.
Plath was the lone occupant in his vehicle, reported he was not injured in the crash and was wearing his seat belt. The air bag did not deploy in Plath's vehicle. Plath's vehicle sustained very severe damage and was towed from the scene.
