The Dane County Medical Examiner’s Office is releasing the names of the 3 people who died in the three-vehicle crash on 06-19-11. The crash occurred in the 2900 block of USH 12&18 in the Town of Cottage Grove.
A Ford Explorer traveling westbound on USH 12&18 collided, nearly head-on, with a Dodge Caravan that had been traveling eastbound. Both occupants in the Explorer died due to the injuries they sustained in the crash. The driver of the Explorer was:
Darrell Gene REYNOLDS
The passenger in the Explorer was identified as:
Delores Mae REYNOLDS
Darrell and Delores REYNOLDS are husband and wife.
There were two occupants in the eastbound Dodge Caravan. The driver of the van died from the injuries he received in the crash. The driver of the van is identified as:
Wayne A. KOPP
All three decedents were pronounced dead at the scene of the crash. There were no deaths associated with the third vehicle involved in the crash.
This crash remains under investigation by the Dane County Sheriff’s Office and the Dane County Medical Examiner’s Office.

Fatal Crash in the Town of Cottage Grove
Madison, WI. June 20th, 2011 – At approximately 4:29 pm on Sunday, June 19th, Dane County deputies responded to a three-car crash on U.S. Highway 12 & 18, 4/10 of a mile west of County Highway BN in the Town of Cottage Grove.
A 1998 Ford Explorer operated by a 73-year-old Madison man crossed the centerline and struck an eastbound 2005 Buick Rendezvous driven by Thomas Wileman, 74, of Fort Atkinson, Wisconsin. Wileman and his passenger, Carol J. Wileman, 69, were both transported to Fort Atkinson Hospital with non-life threatening injuries.
After striking the Buick Rendezvous, the Ford Explorer struck a 2002 Dodge mini-van that was also traveling eastbound. The driver of the mini-van, a 52-year-old man from Verona, was pronounced dead at the scene. A 17-year-old passenger in the mini-van was transported to U.W. Hospital with non-life threatening injuries. Both occupants of the Ford Explorer were pronounced dead at the scene.
The names of the deceased are being withheld pending notification of the next of kin by the Dane County Medical Examiner. The crash remains under investigation by the Dane County Sheriff’s Office.
It is unknown if speed or alcohol were factors in the crash.
