February 6, 2012 – On Saturday, February 4th, Philip J. Havaich was pronounced dead at UW Hospital. Mr. Havaich died as a result of injuries he sustained in the January 28th single vehicle crash.

From the Dane County Sheriff's Department:
January 30, 2012 – The driver involved in the town of Mazomanie rollover crash is being identified as Phillip J. Havaich, age 51 of Mazomanie.
Mr. Havaich remains hospitalized and alcohol appears to be a factor in the crash.
This incident remains under investigation and a charge for Operating While Intoxicated-4th Offense is pending.

From the Dane County Sheriff's Department:
On Saturday at approximately 9:45 p.m., Dane County Sheriff’s Deputies, along with Mazomanie EMS, Black Earth Fire, and UW-Med Flight, responded to USH 78 at CTH Y, in the Township of Mazomanie for a single vehicle rollover crash.
A preliminary investigation determined that a 51 year-old male, from Mazomanie, was driving a Pontiac Grand Prix traveling southbound on USH 78 when he crossed the center line and over-corrected resulting in the vehicle rolling several times. The driver was not wearing a seat belt and was ejected. The driver was transported to UW-Hospital by Med-Flight with critical, life-threatening injuries. Alcohol appears to be a contributing factor.
This crash remains under investigation by the Dane County Sheriff’s Office.
