UPDATE: 9-year-old killed in multi-vehicle crash in Beaver Dam

UPDATED: Tuesday, August 19, 2014 --- 6:08 p.m.

From the Dodge County Sheriff's Office:

On August 15, 2014 at 4:00 PM, the Dodge County Sheriff’s Office responded to an injury automobile crash on CTH A at the intersection with Indian Hills Trail in the Town of Beaver Dam.

Investigation revealed that a 2010 Honda hatchback was traveling south on Cth A and slowing to conduct a turn onto Indian Hills Trail.

A tractor/trailer rig was following the Honda, but the tractor/trailer failed to slow for the Honda. The front of the tractor/trailer struck the rear of the Honda, forcing it into the northbound traffic lane where it struck a northbound Dodge passenger car.

The driver of the tractor/trailer (Lee Henricksen, 67 years old, from Fox Lake) was uninjured in the crash. The driver of the Dodge (Jennifer Polenska, 31 years old, from Waupun) was transported to Beaver Dam Hospital and later transferred to UW Hospital with non-life threatening injuries.

The driver of the Honda (Judith Haddad, 45 years old, from Lindenhurst, IL) sustained non-life threatening injuries and was flown from the scene by Flight for Life to UW Hospital. A passenger in the Honda (Ilan Hurtado, 9 years old, from Lindenhurst, IL) sustained severe injuries and was flown to UW Hospital by another Flight for Life helicopter.

Assisting at the scene were Beaver Dam Police and Fire Departments, Beaver Dam and Horicon EMS, 2 Flight for Life helicopters and DCERT.

The crash remains under investigation by the Dodge County Crash Investigation Team.

Ilan Hurtado, 9 years old, from Lindenhurst, IL died as results of injuries received.

Three people are left injured, one uninjured after a multi-vehicle crash in the Town of Beaver Dam.

The Dodge County Sheriff's Office responded to a crash on County Trunk Highway A at the intersection with Indian Hills Trails in Beaver Dam. A 2010 Honda hatchback was traveling south on CTH A and slowed down to turn onto Indian Hills Trail. A semitrailer was behind the Honda hatchback and struck the rear end of the vehicle, forcing it into the northbound traffic lane. It struck a Dodge passenger car.

The driver of the semitrailer, 67-year-old Lee Henricksen from Fox Lake, was uninjured in the crash. The driver of the Dodge, 31-year-old Jennifer Polenska from Waupun, was transported to Beave Dam Hospital, later transferred to UW Hospital with non-life threatening injuries.

The driver of the Honda, 45-year-old Judith Haddad from Lindenhurst, IL., sustained non-life threatening injuries and was flown from the scene by Flight for Life to UW Hospital. The passenger in the Honda, 9-year-old Illan Hurtado from Lindenhurst, IL., sustained severe injuries and was flown to UW Hospital by another Flight for Life helicopter.
