Portage Police have identified the man killed in an industrial accident at Wieser Concrete as 29-year-old Michael Kittle of Pardeeville.
Officials say a cement form fell on Kittle Friday at the Riley Rd. business.

According to the Columbia County Sheriff's Office, Occupational Safety & Health Administration is investigating the death of a Wieser Concrete employee in Portage.
On Friday, February 28th, at 2:00 p.m., police, fire, and EMS responded to the business located on Riley Rd. on the west side of Portage for an industrial accident. Officials said a cement form had fallen on an employee.
Upon arrival, it was discovered that the employee was deceased, according to a release. Police are not releasing the name of the victim at this time until all next of kin can be notified.
According to a release, an initial investigation, it appears that some of the supports or “lifters” connected to the support chains had pulled away from the heavy cement form causing the form to drop.
OSHA was notified and two of their investigators from the Madison office responded. They will be continuing their investigation early next week, according to the release. The Portage Police Department was assisted by the Portage Fire Department, Divine Savior EMS and the Columbia County Medical Examiner’s Office.
